Download and Complete the Medical History Form
Please download and complete the Medical History Form to provide us with your past and present medical history. This information is important for us to be able to safely care for your dental health needs, so please be as complete and accurate as you can when providing this information.
Any Special Medical Conditions?
For patients with special medical conditions, it’s highly advisable to bring your current medical laboratory results with you as well as a list of all the medications you are currently taking. In some instances, it may be necessary to contact your physician to clarify questions we may have regarding your health status. We do this for your safety and well being.
Taking Antibiotic Prophylaxis?
If you routinely take antibiotic prophylaxis prior to dental appointments, please take them as prescribed before coming. If you are unsure if you need antibiotic prophylaxis, you should still come for your first appointment, but please do be aware that we may not be able to complete the entire comprehensive examination on that day.
Bring Any Referral Notes
If you have been referred to our clinic by another office it can be very helpful to bring a referral note with the dentist’s name and phone number. In some instances we may be able to use current x-rays from other offices, however; it may be necessary for us to make additional x-ray images of your teeth in order to properly identify any dental problems you may have.
